I primarily accept insurance and L&I clients at Paul Perrotta and Associates* at Seattle Healing Arts in Ravenna near Whole Foods.   I am also doing a limited amount of insurance work out of my private practice (PIP, Premera, Aetna, Cigna, WholeHealth Network). 

I do deep tissue and Swedish massage for insurance work.  Once in awhile I might do a few strokes of Ashiatsu, if I determine that it would be the most effective modality and if the client is open to receiving that work.  I do not do full AOBT sessions for insurance work. 

If you wish to schedule a series of insurance massages at my private practice, I will need to get detailed information from you including a copy of the front and back of your insurance card, a dated, valid prescription including the frequency and duration of your treatment and diagnosis codes for  your condition.  Even if your insurance says that you can self-refer, you still need to bring in a current diagnosis code (within the last six months), or I will be unable to bill your insurance.  It is out of my scope of practice to give you a diagnosis code, so this must come from your physician. 

Please provide me with the following details of your insurance coverage: how many massages you are allowed (and how many you have used), your deductible, your co-pay, and the name of the person who is covered if you are not the primary policy holder.  Please also check to see if I am in network for your plan.  If I am out of network, you must pay for the massage and get reimbursed by your insurance.  I will provide you with detailed receipts.    If you receive massage from me and it turns out that you are not eligible for insurance massage, you will be responsible for paying me out of pocket at my hourly rate, which is $70.  Make sure to bring your calendar with you to your first massage so that we can schedule weekly sessions for the duration of your prescription.
